By signing this Policy Acknowledgement Form, you confirm that you have read, understood, and agree to comply with all OTF Fitness Center policies listed below. These Terms and Conditions govern your participation in classes, sessions, and use of our facilities.
1. Acknowledgement of Policies
You confirm that you have reviewed and agree to abide by:
- OTF Fitness Center Booking Policy
- Cancellation & Rescheduling Policy
- Code of Conduct & Facility Use Policy
- Health Screening & Medical Clearance Policy
- Liability Waiver & Release Policy
- Privacy & Data Protection Policy
2. Membership and Participation
- You must meet all membership eligibility requirements and complete any required enrollment forms.
- You are responsible for your personal belongings; OTF Fitness Center is not liable for lost or stolen items.
- Participation in any class or training session is voluntary, and results may vary based on individual effort and adherence.
3. Assumption of Risk & Release
- You acknowledge that exercise and use of fitness equipment carry inherent risks of injury.
- You assume all risks associated with participation and agree to hold OTF Fitness Center, its owners, staff, and affiliates harmless from any claims, damages, or liabilities.
- This release applies to claims arising from negligence, equipment failure, or facility conditions.
4. Policy Modifications
OTF Fitness Center reserves the right to update or modify any policy at its discretion. Changes take effect 30 days after posting on our website and in-facility notices. Continued participation constitutes acceptance of the revised policies.
5. Governing Law
These Terms and Conditions are governed by the laws of South Carolina. Any dispute arising under these terms will be resolved through binding arbitration in Marlboro County, South Carolina.
